1. Identification Form for Genetic Testing

Draw all white markings, brands & mark whorls on head and neck with an X.

Please send this completed signed form to: AQHA, PO Box 979, Tamworth NSW 2340

2. DNA/ Genetic Test Kit Form

   Check information in the box (top right) are the correct details for your horse

Select mane hair well forward of the withers, or tail hair in foals.

Wrap the hairs around your finger or a comb, as close as possible to the skin, pull firmly and swiftly upward to ensure you obtain the hair roots (follicles).

Pull the hair out at the roots and keep dry. Do not touch the hair roots.

Make sure you have around 30-40 hair roots. 

Place the hair shafts on the hair sample box on the Genetic Test Kit Form.

Make sure the roots of the hair roots (follicles) are sitting on the box marked 1 (to the right).

Trim excess hair shafts in area marked 4. Do not trim the hair roots off.

Fold this document where indicated and place into a re-sealable plastic bag, seal the bag.

Only put one hair sample in the plastic bag and do not put more than 3 plastic bags in the one envelope.

Place sealed plastic bag into an envelope and send to: EGRC, PO Box 433, Scone NSW, 2337
