AQHA Forms
Registration

AQHA forms are subject to change at any point in time, and should be checked for validity before sending them to the Association.
They are provided in PDF format, if you do not have Adobe Acrobat Reader, you can download it free from here. Once you have installed Adobe Acrobat Reader, you are able to view and print out these forms, and fill them in. Once completed, you can send them by post along with payment to the Association.
Please note that fax copies of some forms are not acceptable. Always check with the office prior to faxing a form for the first time.

PLEASE NOTE: Application for Registration should come from the stallion owner once all commercial obligations have been met. For registrations with yellow Breeding Certificate please use below Application for Registration.
