AQHA Forms
Memberships

AQHA forms are subject to change at any point in time, and should be checked for validity before sending them to the Association.
They are provided in PDF format, if you do not have Adobe Acrobat Reader, you can download it free from here. Once you have installed Adobe Acrobat Reader, you are able to view and print out these forms, and fill them in. Once completed, you can send them by post along with payment to the Association.
Please note that fax copies of some forms are not acceptable. Always check with the office prior to faxing a form for the first time.

Membership

The Online Membership Renewal facility for 2010/2011 will be available for all members that have been financial during the last 2 membership years.

Life, Honorary Life and overseas Members cannot renew online and are asked to return the signed and dated Renewals by return mail or fax.
